ABSTRACT

RESUMEN Este estudio tiene como objetivo evaluar la prevalencia del dolor cervical y los factores asociados entre agricultores que producen tabaco. Se realizó un estudio transversal en el que participaron 2.469 agricultores que producen tabaco en el sur de Brasil. Para la caracterización del dolor cervical se utilizó una adaptación del cuestionario nórdico para síntomas musculoesqueléticos. El análisis multivariante se realizó mediante la regresión de Poisson, siguiendo un modelo teórico jerárquico. La prevalencia del dolor cervical en el año previo entre la población estudiada fue del 7,4%. Las variables que se asociaron con el dolor cervical entre las mujeres trabajadoras fueron la edad, el consumo de tabaco, el enfardado del tabaco, el uso de motosierras pesadas, trabajar en un ritmo intenso o acelerado y la enfermedad del tabaco verde, mientras que, entre los varones, fueron la edad, el uso de motosierras pesadas, el trabajo sentado en el suelo, la intoxicación por plaguicidas y la enfermedad del tabaco verde. El estudio refuerza la importancia de las cargas de trabajo ergonómicas y fisiológicas en la determinación del dolor cervical. Se necesitan estudios futuros para comprender el papel de la exposición a los plaguicidas y a la nicotina en los problemas musculoesqueléticos. La mecanización de la cosecha del tabaco podría reducir la exposición ergonómica y química, mejorando así la salud de los agricultores.


ABSTRACT This study aims to assess neck pain prevalence and associated factors among tobacco farm workers. This is a cross-sectional study of 2,469 tobacco farm workers in southern Brazil. An adapted version of the Nordic Questionnaire of Musculoskeletal Symptoms was used to characterize neck pain. Multivariate analysis was performed using Poisson regression, following a hierarchical theoretical model. Neck pain prevalence in the last year among the population studied was 7.4%. Worker age, tobacco smoking, tobacco bundling, use of heavy chainsaws, working at an intense or accelerated pace and green tobacco sickness were variables associated with neck pain in females. Among males, age, use of heavy chainsaws, working in a sitting position on the ground, pesticide poisoning, and green tobacco sickness were associated with the outcome. The study reinforces the importance of ergonomic and physiological workloads in the determination of neck pain. Future studies are needed to understand the role of pesticides and nicotine exposures on musculoskeletal problems. The mechanization of tobacco harvesting could reduce ergonomic and chemical exposure, thereby improving farmers' health.

ABSTRACT

Introduction: Agrochemicals, also known as pesticides, are widely used in agriculture and in public health. They are organic and inorganic chemical substances with a high level of toxicity not only for the environment, but also for human health. Objective: To verify findings on labyrinthine assessment in endemic disease control agents, and to recommend the inclusion of the vestibular exam in the set of tests for pesticide-exposed populations. Methods: Descriptive, prospective, cross-sectional study with a sample comprising 15 endemic disease control agents, males, mean age of 51.6 years old (standard deviation [SD] = 5.9). All of the participants were submitted to anamnesis, otorhinolaryngological screening, and vestibular assessment. Results: Regarding the most reported complaints, dizziness (73.4%), headache (60%), and tingling in the extremities (53.4%) were observed. The findings of the vestibular exams were normal in 53.3%, while 46.7% showed peripheral vestibular disorder, of which 26.7% were of deficitary type, and 20% of the irritative type. Conclusions: Alteration in the vestibular system was verified in 50% of the workers, with a greater prevalence in the caloric testing. Several disorders related to pesticides intoxication are scientifically known. Actions promoting knowledge and qualification of this population for the proper handling of chemicals are suggested, in addition to the elaboration and inclusion of protocols of vestibular assessment in hearing health programs for the prevention, diagnosis and treatment of vestibular disorders (AU)

Antecedentes: El uso de plaguicidas se ha incrementado a partir de los años 40's con la introducción de las sustancias sintetizadas químicamente. El uso de estos va de la mano con el crecimiento constante de la economía agraria. Los primeros plaguici - das utilizados fueron los Organoclorados, en los años 50's fueron sustituidos por Organofosforados y Carbamatos, actualmente existe una amplia variedad de agroquímicos con diferentes grados de toxicidad. Objetivo. Caracterizar los casos notiicados de intoxicación por plaguicidas en Hospital de Área Enrique Aguilar Cerrato en el Departamento de Intibucá. Material y métodos. Se revisaron 232 expedientes de pacientes con intoxicación aguda por plaguicidas, tratados en la emergencia de enero del 2010 a diciembre del 2014. Se recopilaron variables demográicas, clínicas y laboratoriales mediante icha de recolección de datos y se aplicó estadística descriptiva. Resultados. Predominó el sexo masculino 187 casos (81%) y la edad media fue de 26 años, siendo la población agri - cultora la más afectada (68%). El modo de intoxicación más frecuente fue la intencional (74%). Involucrados los bipiridilos (33%) y organofosforados (30%) predominantemente. Discusión. Las intoxicaciones agudas por plaguicidas en Intibucá son más frecuentes en los agricultores masculinos alfabetas y el intento suicida por vía oral la causa principal de las mismas. Los plaguicidas más impli - cados son los bipiridilos y los organofosforados, ocupando el primer y segundo lugar respectivamente. No hubo muertes reportadas en nuestro estudio...(AU)

ABSTRACT

O objetivo deste estudo foi construir um instrumento que avalia a qualidade de vida no trabalho de forma global, seguindo os moldes dos instrumentos de avaliação da qualidade de vida da Organização Mundial da Saúde (WHOQOL) e alicerçado nos modelos teóricos clássicos de qualidade de vida no trabalho, com direcionamento para a sociedade contemporânea brasileira. Metodologia A validação de conteúdo foi realizada através da análise por pesquisadores da área da qualidade de vida no trabalho, enquanto a verificação da consistência interna ocorreu por meio da utilização do coeficiente alfa de Cronbach, em uma aplicação a 143 indivíduos. Resultados A versão final do instrumento é constituída de 47 questões, sendo cinco para conhecimento da amostra e 42 seccionadas em cinco esferas que contemplam as dimensões da qualidade de vida no trabalho. O alfa de Cronbach obtido a partir da aplicação do instrumento foi de 0,8568. Para o cálculo dos resultados do instrumento fora desenvolvida a sintaxe SPSS e uma ferramenta no software Microsoft Excel que realiza os cálculos forma automatizada após a tabulação dos dados. Conclusão Conclui-se que o objetivo de validar um instrumento global de avaliação da qualidade de vida no trabalho validado a partir da cultura hodierna brasileira, com características psicométricas satisfatórias foi atingido, podendo este ser aplicado sem a obrigatoriedade de utilização do software SPSS...


The objective of this study was to build an instrument that evaluates quality of working life (QW:) in a comprehensive way that is in line with the WHOQOL instruments and is based on QWL's classic theoretical models, directed toward contemporary Brazilian society. Methods The content validation was performed through analysis by researchers in the area of QWL, and the verification of internal consistency was performed with Cronbach's alpha. The instrument was administered to 143 individuals. Results The final version of the instrument consists of 47 questions, with five for sampling knowledge and the remaining 42 divided into five spheres that take into account the dimensions of QWL.. Cronbach's alpha obtained from the administration of the instrument was 0.8568. A SPSS syntax and a tool in Microsoft Excel that performs the automated calculation after data tabulation was developed to calculate the results. Conclusions We conclude that the goal of validating a comprehensive instrument with satisfactory psychometrical characteristics for the evaluation of QWL based on contemporary Brazilian culture was reached. This tool may be used without a need for SPSS utilization...

ABSTRACT

Objetivos . Estimar la seroprevalencia de leptospirosis en agricultores dedicados al cultivo de arroz del valle del Alto Mayo, región San Martín e identificar anticuerpos para los serovares circulantes y los factores asociados a la infección. Materiales y métodos. Se realizó un estudio transversal entre octubre y noviembre de 2010 en una muestra de 254 agricultores procedentes de las once comisiones de regantes del valle de Alto Mayo. Se recolectó una muestra de sangre y se aplicó un cuestionario que incluyó variables sociodemográficas, y características del hogar y del trabajo. Se determinó la presencia de anticuerpos contra leptospira usando la prueba de microaglutinación y ELISA IgM. Resultados. La seroprevalencia determinada por microaglutinación fue del 64,6% (IC 95%: 58,6-70,6). La prueba ELISA IgM fue reactiva en 15,0% (IC 95%: 10,5-19,5). Los tipos de serovar más frecuentes fueron Icterohaemorrhagiae, Autumnalis, Australis , Panama y Grippotyphosa. Los factores asociados independientemente fueron la manipulación de roedores (OR ajustado [ORa] de 7,9, IC 95%: 1,6-37,9), trabajar descalzo (ORa de 2,9, IC 95%: 1,2-6,8) y el sexo masculino (ORa de 4,5, IC 95%: 1,3-15,3). Conclusiones . El valle del Alto Mayo es un área hiperendémica de leptospirosis. Los factores asociados más importantes fueron la práctica de trabajar descalzo y la de manipular roedores en los campos de cultivo. En consecuencia, se recomienda implementar intervenciones que incidan en estos dos factores a fin de reducir el riesgo de infección en los cultivadores de arroz.


Objectives. To estimate the seroprevalence of leptospirosis in rice farmers of the Alto Mayo Valley in the Peruvian region of San Martín and to identify factors associated with infection. Materials and methods. A cross-sectional study was conducted between October and November of 2010 on a sample of 254 farmers from eleven irrigation committees in the Alto Mayo Valley. A blood sample was collected as well as sociodemographic, household and labor characteristics in the form of a questionnaire. The presence of antibodies against leptospira was determined using the microscopic agglutination test and the IgM ELISA. Results. The seroprevalence by agglutination was 64.6% (95% CI: 58.6-70.6). The IgM ELISA test was reactive in 15,0% (95% CI: 10.5-19.5). The most frequent serovar were Icterohaemorrhagiae, Autumnalis, Australis, Panama and Grippotyphosa. Independently associated factors were handling rodents (adjusted OR (aOR): 7.9; 95% CI: 1.6-37.9), working barefoot (aOR: 2.9; 95% CI: 1.2-6.8) and male sex (aOR: 4.5; 95% CI: 1.3-15.3). Conclusions. The Alto Mayo Valley is a hyperendemic for leptospirosis. The most important factors were associated with working barefoot and handling rodents in crop fields. Consequently, it is recommended to implement interventions that affect these two factors in order to reduce the risk of infection in rice farmers.

ABSTRACT

A cromomicose é uma micose profunda, crônica, com acometimento da pele e do subcutâneo. O fungo é encontrado na natureza nas plantas e no solo, sendo introduzido no organismo através de traumas ou ferimentos. A localização das lesões é, principalmente, nos membros inferiores, podendo também comprometer outras regiões. Os trabalhadores rurais são mais frequentemente acometidos por falta de proteção e exposição contínua. O objetivo deste relato foi apresentar um caso de cromomicose de evolução crônica, fazendo diagnóstico diferencial com outras doenças que causam a síndrome verrucosa (leishmaniose, esporotricose e tuberculose) e realizando breve revisão da literatura. Paciente do gênero masculino, 83 anos, branco, trabalhador rural, procurou Ambulatório de Dermatologia apresentando lesão vegetante, verrucosa, de base eritematosa, com distribuição linear, localizada no membro superior direito com evolução de 9 anos. As características da lesão sugeririam como hipóteses diagnósticas doenças que causam a síndrome verrucosa LECT (iniciais de leishmaniose, esporotricose, cromomicose e tuberculose). Para confirmação diagnóstica, foi realizada biópsia da lesão e exame histopatológico, que revelou presença de células arredondadas de cor castanho escuro em processo de reprodução binária, confirmando o diagnóstico de cromomicose. O paciente foi submetido ao tratamento com itraconazol e crioterapia combinados. A cromomicose possui diagnósticos diferencias em decorrência das características clínicas da lesão,sendo fundamentais, para sua confirmação diagnóstica, exames específicos.


Chromomycosis is a deep and chronic mycosis that affects the skin and the subcutaneous tissues. The fungus is found in nature, in plants and soil, being introduced into the body through trauma or injury. Lesions occur mainly on the lower limbs, but can also involve other regions. Rural workers are more frequently affected due to lack of protection and continuous exposure. The objective of this report was to present a case of chronic chromomycosis evolution, making differential diagnosis with other diseases that cause warty syndrome (leishmaniasis, sporotrichosis and tuberculosis) and performing a brief literature review. Male patient, 83 years old, white, farm laborer, sought the Dermatology Department presenting vegetative, verrucous lesion, with erythematous base, linear distribution, located on the right upper limb, with a 9-year progression. Lesion characteristics would suggest as diagnostic hypotheses diseases that cause warty syndrome LECT (acronym in Portuguese from leishmaniasis, sporotrichosis, chromomycosis and tuberculosis). For diagnostic confirmation, a biopsy and an histopathological examination were performed and revealed the presence of round, dark brown cells in binary reproduction process, confirming the diagnosis of chromomycosis. The patient was treated with itraconazole and cryotherapy combination. Chromomycosis has differential diagnosis due to the clinical characteristics of the lesion; therefore specific tests are fundamental to confirm the diagnosis.

ABSTRACT

Streptococcus suis causes meningitis, bacteremia, endocarditis, endophthalmitis, artrithis and toxic shock in both pigs and human beings. The incidence of this pathogen is increasing worldwide. A case of 54 years old men from a rural zone of Tucumán, Argentina with two days of headache, vomit and photophobia was admitted septic, with purulent meningitis. Streptococcus suis was growth from the blood and cerebrospinal fluid culture. The patient was treated with ceftriaxona and had a good evolution without sequels. This is the first case of S. suis described in the northwest of Argentina and the third of Latin America, and it is emphasized that the occupational contact with pigs is an important background for the initial suspect of this pathogen.


La infección por Streptococcus suis puede causar meningitis, bacteriemia, endocarditis, endoftalmitis, artritis y shock tóxico, tanto en el ser humano como en cerdos. La incidencia de esta zoonosis está aumentando en el mundo. Se presenta el caso de un varón de 54 años de edad, trabajador rural proveniente de la provincia de Tucumán, Argentina, que consultó por cefalea, vómitos y fotofobia de dos días de evolución, constatándose una sepsis de foco meníngeo. Tanto en el cultivo de líquido cefalorraquídeo como en el hemocultivo se aisló S. suis. Se trató con ceftriaxona con buena evolución y sin secuelas. Es el primer caso de S. suis en el noroeste argentino y el tercero en Latinoamérica. Se resalta la importancia del antecedente de exposición laboral a cerdos para la sospecha inicial de este microorganismo.

ABSTRACT

Human infection by Streptococcus suis is a zoonosis with a known occupational risk. Meningitis is its most frequent clinical manifestation. We present the first two cases in Chile. First case: 54-year-old female patient, pig-farmer. She presented headache, vomiting, confusion and meningismus. She presented septic shock. Second case: 48-year-old male patient, also pig farmer, presented headache, vomiting and meningismus. A Gram's staining of cerebrospinal fluid (CSF) showed gram-positive cocci in both cases. Ceftriaxone and dexamethasone treatment was administered. The CSF cultures were positive for Streptococcus suis serotype 2. The patients experienced a good outcome, without neurological sequelae at the time of discharge. It is considerable to evaluate epidemiologic factors in order to suspect this etiological agent in cases of meningitis. These cases enhance the need of heighten awareness of potential for occupational exposure and infection by this emerging human pathogen. Educating population at risk about simple preventive measures must be considered.


La infección humana por Streptococcus suis es una zoonosis con riesgo ocupacional conocido, siendo la meningitis aguda su manifestación clínica más frecuente. Se presentan los dos primeros casos en Chile. Primer caso: Mujer de 54 años con un cuadro de cefalea y vómitos, confusión y signos meníngeos. Evolucionó con un shock séptico. Segundo caso: Varón de 48 años, refirió cefalea y vómitos. Presentó signos meníngeos al examen físico. En ambos casos en la tinción de Gram de líquido cefalorraquídeo (LCR) se observaron cocáceas grampositivas. Fueron tratados con ceftriaxona y dexametasona. El cultivo de LCR fue positivo en ambos casos para S. suis serotipo 2. En los dos pacientes la evolución clínica fue favorable, sin alteraciones neurológicas al alta. En ambos casos se obtuvo en forma retrospectiva el antecedente de realizar labores de crianza de ganado porcino. Se destaca la importancia de investigar los antecedentes epidemiológicos para sospechar este agente etiológico en meningitis aguda. Se debe considerar el riesgo ocupacional en una posible infección por este patógeno humano emergente y educar a la población en riesgo sobre medidas preventivas simples.

OBJETIVO: Estimar o coeficiente de mortalidade por intoxicações ocupacionais relacionadas aos agrotóxicos no Brasil. MÉTODOS: Utilizaram-se dados do Sistema de Informações sobre Mortalidade de 2000 a 2009 referentes aos diagnósticos de intoxicação por agrotóxicos, T60.0-T60.4, T60.8 e T60.9, X48, Y18, e Z578 da CID-10, para a causa básica ou associadas; a natureza ocupacional foi identificada pelo registro no campo <acidente de trabalho>, <circunstância do óbito> e se a <ocupação> era na agropecuária. Foram excluídos homicídios e suicídios. Para cálculo da mortalidade, o número de trabalhadores da agropecuária foi obtido do Instituto Brasileiro de Geografia e Estatística, projeções do Sistema de Contas Nacionais. RESULTADOS: Foram encontrados registros de 2.052 óbitos por intoxicação por agrotóxicos no Brasil, dos quais 36,2% (n = 743) não continham dados sobre a ocupação. Dentre os 1.309 restantes, 679 (51,9%) eram trabalhadores da agropecuária. A mortalidade por intoxicação ocupacional por agrotóxicos caiu de 0,56/100.000 (2000 a 2001) para 0,39/100.000 (2008 a 2009) entre trabalhadores no período, maior queda entre os homens que entre as mulheres. Os homens tiveram maiores estimativas de mortalidade por esse tipo de intoxicação em todos os anos. A maior parte dos óbitos foi causada por agrotóxicos do tipo organofosforados e carbamatos. O número de óbitos por esse tipo de intoxicação declinou em todas as regiões, exceto no Nordeste. CONCLUSÕES: É necessária a melhoria dos registros das declarações de óbito, em especial da ocupação e da relação dos diagnósticos com o trabalho, fundamentais para o controle e prevenção ...

OBJECTIVE: To estimate the mortality rate due to occupational pesticide poisoning in Brazil. METHODS: Data on diagnoses of death from pesticide poisoning between 2000 and 2009 were obtained from the Mortality Information System. ICD-10 codes T60.0-T60.4, T60.8 and T60.9, Y18, X487 and Z578 as the main or secondary cause of death; data on work-related deaths were obtained from the death certificate, from the fields <work related accident>, <circumstances of death> and whether cases were agricultural workers. Homicides and suicides were excluded. To calculate mortality, the number of agricultural workers was obtained from the Brazilian Institute of Geography and Statistics, National System of Accounts estimates. RESULTS: There were 2,052 deaths recorded as caused by pesticide poisoning in Brazil, between 2000 and 2009, of which 36.2% (n = 743) had no occupation data. Of the remaining 1,309, 679 (51.9%) were agricultural workers. Mortality from occupational pesticide poisoning declined from 0.56/100.000 (2000-2001) to 0.39/100.000 (2008-2009) workers during the study period, and there was a larger decrease among men compared with women. Males had a higher mortality from this type of poisoning than women in all study years. Most deaths were caused by organophosphates and carbamate pesticides poisoning. During the study period the number of cases declined in all regions, except for the Northeast. CONCLUSIONS: Improvement in the quality of Death Certificate records is needed, particularly for occupation and the assessment of causes of death as work related, crucial for work injuries control and prevention programs. Special attention is required in the Northeast region. .

ABSTRACT

We tested sera from 286 agricultural workers and 322 rodents in the department of Córdoba, northeastern Colombia, for antibodies against two hantaviruses. The sera were analysed by indirect ELISA using the lysate of Vero E6 cells infected with Maciel virus (MACV) or the N protein of Araraquara virus (ARAV) as antigens for the detection of antibodies against hantaviruses. Twenty-four human sera were IgG positive using one or both antigens. We detected anti-MACV IgG antibodies in 10 sera (3.5%) and anti-ARAV antibodies in 21 sera (7.34%). Of the 10 samples that were positive for MACV, seven (70%) were cross-reactive with ARAV; seven of the 21 ARAV-positive samples were cross-reactive with MACV. Using an ARAV IgM ELISA, two of the 24 human sera (8.4%) were positive. We captured 322 rodents, including 210 Cricetidae (181 Zygodontomys brevicauda, 28 Oligoryzomys fulvescens and 1 Oecomys trinitatis), six Heteromys anomalus (Heteromyidae), one Proechimys sp. (Echimyidae) and 105 Muridae (34 Rattus rattus and 71 Mus musculus). All rodent sera were negative for both antigens. The 8.4% detection rate of hantavirus antibodies in humans is much higher than previously found in serosurveys in North America, suggesting that rural agricultural workers in northeastern Colombia are frequently exposed to hantaviruses. Our results also indicate that tests conducted with South American hantavirus antigens could have predictive value and could represent a useful alternative for the diagnosis of hantavirus infection in Colombia.

PURPOSE: The objective of this research was to assess the ordering and temporal resolution auditory abilities in rural workers exposed to pesticides and compare them with laborers exposure index. METHODS: A sectional study assessed 33 individuals of both genders, aged 18-59 years, who were exposed to pesticides during their daily routine. The procedures were: questionnaire, meatoscopy, basic audiological evaluation and Temporal Auditory Processing tests: pattern test duration and Gaps-in-Noise. In order to analyse the results, a variable called 'index of exposure' was set up through a simple sum of variables present in the questionnaire. The tests' results on Temporal Auditory Processing were categorized according to the tercis of distribution, based on the results observed - in this study, tertile 1, tertile 2, and tertile 3 - and then compared with the exposure index. RESULTS: Difference was verified in all tertiles, with a dose-response relationship, i.e. increased average exposure was associated to worse performance on pattern test duration (p=0.001) and Gaps-in-Noise (p=0.001) in all tertiles. The highest correlation was observed between tertiles 3 and 1. CONCLUSION: Workers exposed to pesticide performed bellow average on Temporal Auditory Processing tests. There was association between the index of exposure to pesticides and worse performance in Temporal Auditory Processing tests, suggesting that the pesticides may be harmful to central auditory pathways.


OBJETIVO: Investigar as habilidades auditivas de ordenação e resolução temporal, em trabalhadores rurais expostos ocupacionalmente a agrotóxicos, e correlacionar estes resultados com o grau de exposição dos laboriosos a estas substâncias. MÉTODOS: Foi realizado um estudo seccional, por meio da avaliação de 33 indivíduos de ambos os gêneros, com idades entre 18 e 59 anos, expostos ocupacionalmente a agrotóxicos. Aplicou-se os seguintes procedimentos: questionário, meatoscopia, audiometria, imitanciometria e testes do Processamento Auditivo Temporal: Teste de Padrão de Duração e Gaps-in-Noise. Para análise dos resultados criou-se uma variável denominada índice de exposição, por meio de um somatório de variáveis presentes no questionário. Os resultados dos testes de Processamento Auditivo Temporal aplicados foram categorizados segundo os tercis de distribuição, de acordo com o resultado observado - sendo neste estudo denominado de Tercil 1, Tercil 2 e Tercil 3 - e então, comparado com o índice de exposição. RESULTADOS: Verificou-se diferença em todos os tercis, havendo relação dose-resposta: conforme foi aumentada a média de exposição, pior foi o desempenho no Teste de Padrão de Duração (p=0,001) e no Gaps-in-Noise (p=0,001), em todos os tercis. A maior correlação foi observada entre o Tercil 3 e o Tercil 1. CONCLUSÃO: Os trabalhadores expostos ao agrotóxico apresentaram desempenho inferior ao esperado para os padrões de normalidade nos testes de Processamento Auditivo Temporal. Houve associação entre o índice de exposição a agrotóxico e pior desempenho nos testes de Processamento Auditivo Temporal, sugerindo que o agrotóxico pode ser uma substância nociva às vias auditivas centrais.

INTRODUCTION: According to reports by the Ministry of Health, in the far western region of the State of Santa Catarina, there have been no reports of hantavirus pulmonary syndrome, a zoonotic disease transmitted by feces of infected rodents. A seroepidemiological study of residents of this region, was conducted, with the aim of determining the presence of hantavirus infections. A total of 340 volunteers of both genus, from the towns of Belmonte and Paraíso, were studied. METHODS: The serum of these patients was collected and used to detect IgG antibodies against recombinant N protein of Araraquara hantavirus, by ELISA assay. The positive samples were then titrated and confirmed by immunofluorescence assay. RESULTS: This study demonstrated the presence of IgG antibodies against hantavirus N protein in 3.5 percent of the population. The most frequent occupation was farm worker, 81 percent had direct and indirect contact with rodents, 91.7 percent of positive cases were farm workers, indicating that the probable cause of infection occurred during barn cleaning. These antibodies are noteworthy, given that the levels of antibodies were verified in individuals whose contact with hantavirus may have occurred many years ago. CONCLUSIONS: This study shows the circulation of hantavirus in the region, a fact that until now, had not reported. All the serum reagents had contact with the pathogen, but did not develop pulmonary and cardiovascular syndrome. It is important to remain alert, because hantavirus is a serious and emerging disease of some relevance.


INTRODUÇÃO: De acordo com relatórios do Ministério da Saúde, na região do extremo oeste do Estado de Santa Catarina, não há relatos de síndrome pulmonar por hantavírus, doença zoonótica transmitida por excretas de roedores contaminados. Com a finalidade de demosntrar a infecção por hantavírus, um estudo soroepidemiológico de moradores da região foi realizado. Assim, foi estudado um total de 340 voluntários de ambos os gêneros, dos municípios de Belmonte e Paraíso. MÉTODOS: O soro destes pacientes foi coletado e usado para a detecção de anticorpos IgG contra a proteína N recombinante de hantavírus Araraquara, pelo teste de ELISA. As amostras positivas foram tituladas e confirmadas por imunofluorescência indireta. RESULTADOS: Este estudo demonstrou a presença de anticorpos IgG contra a proteína N hantavírus em 3,5 por cento da população. A ocupação de lavrador foi a mais frequente, e 81 por cento tiveram contato direto e indireto com os roedores, 91,7 por cento dos casos positivos foram os agricultores, a causa provável da infecção foi através da limpeza de celeiros. Estes anticorpos são notáveis, dado que os níveis de anticorpos são encontrados nos indivíduos cujo contato com o hantavírus pode ter ocorrido há muitos anos. CONCLUSÕES: Este estudo mostra a circulação de hantavírus na região, um fato que até então não havia relatado, todos os reagentes soro tiveram contato com o patógeno, mas não desenvolveram a síndrome pulmonar e cardiovascular. É preciso estar alerta, porque é uma doença grave e emergente com grande importância.

The various kinds of work may interfere in the laborer's quality of sleep. Among the factors, the unemployment caused by the period between the coffee harvests on the agriculture of South Minas may influence the appearance of anxiety and depression symptoms, and it may cause impairments to the quality of life and changes in the rural laborers' pattern of sleep. OBJECTIVE: To appraise the sleep and the effects on the quality of life in the rural laborers in the period between the coffee harvests. METHOD: In the study-group, 40 patients, rural laborers, were evaluated, and in the control-group, 40 patients, fixed laborers. Instruments used: clinical interview, The Pittsburgh Sleep Quality, Index Beck Anxiety Inventory, Hamilton Depression Rating Scale, and World Health Organization Quality of Life. RESULTS: The study-group showed impairment in the quality of the sleep; anxiety and depression signals and symptoms were high in both groups; being them higher in the harvesters laborers; the quality of life presented good levels in both groups, with losses in the domain "social relationship" between the harvesters; the sleep disturbances influenced the appearance of anxiety and depression signs and symptoms, but they did not interfere in the perception of the quality of life. CONCLUSION: The rural laborers that have fixed labors are less exposed to the symptoms of anxiety and depression - along with sleep disorders - than the harvesters laborers. Thus, the study-group's perception of the quality of life is better than the control-group's.


As diversas formas de trabalho podem interferir na qualidade do sono do trabalhador. Entre os fatores, o desemprego ocasionado pela entressafra da cultura cafeeira nas lavouras sul mineiras pode influenciar no surgimento de sintomas de ansiedade e de depressão e causar prejuízos na qualidade de vida e alterações no padrão do sono dos trabalhadores rurais. OBJETIVO: Avaliar o sono e as repercussões na qualidade de vida em trabalhadores rurais na entressafra cafeeira. MÉTODO: No grupo-estudo foram avaliados 40 pacientes trabalhadores rurais e no grupo-controle 40 pacientes trabalhadores fixos. Instrumentos utilizados: entrevista clínica, The Pittsburgh Sleep Quality Index, Beck anxiety Inventory, Hamilton depression rating Scale e World Health Organization Quality of Life. RESULTADOS: O grupo-estudo apresentou prejuízo na qualidade do sono; sinais e sintomas de ansiedade e depressão foram elevados nos dois grupos, sendo maior nos trabalhadores safristas; a qualidade de vida apresentou bons níveis nos dois grupos, havendo perdas no domínio "relações sociais" entre os safristas; os distúrbios do sono influenciaram no surgimento de sinais e sintomas de ansiedade e depressão, mas não interferiram na percepção da qualidade de vida. CONCLUSÃO: Trabalhadores rurais detentores de trabalho fixo estão menos expostos aos sintomas de ansiedade e depressão - acompanhados de distúrbios do sono - que os trabalhadores safristas. Assim, a percepção da qualidade de vida do grupo-estudo é melhor que a do grupo-controle.

A exposição ocupacional e ambiental aos agrotóxicos evidencia-se como um grave problema de Saúde Pública em nosso país. Entre os sintomas apresentados, a tonteira se destaca, em decorrência de uma provável ação tóxica. OBJETIVO: O objetivo do estudo é caracterizar os resultados do exame vestibular de trabalhadores rurais expostos ocupacionalmente e ambientalmente aos agrotóxicos organofosforados. MATERIAL E MÉTODO: Foi realizado um estudo de coorte com corte transversal em 18 trabalhadores rurais de Teresópolis, RJ. A faixa etária variou de 16 a 59 anos com uma média de 39,6 anos, sendo 5 trabalhadores do sexo masculino e 13 do sexo feminino. Foram utilizados três questionários com perguntas relacionadas à saúde geral e auditiva e perguntas específicas à tonteira e suas relações com o trabalho. Todos os trabalhadores passaram por uma avaliação clínica e realizaram os exames vectoeletronistagmografia e audiometria. RESULTADOS: Os resultados mostraram que 16 trabalhadores apresentaram alterações do equilíbrio corporal de tipo periférico irritativo e 7 trabalhadores apresentaram perdas auditivas do tipo neurossensorial, sugerindo que os agrotóxicos induzem alterações do sistema vestibular através de uma intoxicação lenta e silenciosa. CONCLUSÕES: O olhar clínico sobre esta população exposta a substâncias neuro-ototóxicas requer uma abordagem interdisciplinar, assegurando uma intervenção terapêutica e preventiva eficaz.


Occupational and environmental exposure to agricultural pesticides represent an important health care problem in our country. Among the symptoms presented, dizziness stands out, because of a probable toxic action. AIM: The goal of our study was to characterize vestibular test results from rural workers occupationally and environmentally exposed to organophosphates used in agricultural pesticides. MATERIALS AND METHODS: We carried out a cohort cross-sectional study with 18 rural workers from Teresópolis, RJ. Age ranged between 16 and 59 years, with a mean value of 39.6 years, 5 were males and 13 females. We used three questionnaires with questions associated with general and auditory health, and more specific questions about dizziness and its association with work. All workers underwent clinical evaluation, audiometry and vectoelectronystagmography. RESULTS: results showed that 16 workers had irritative peripheral body balance disorder and 7 workers had sensorineural hearing loss, thus suggesting that agricultural pesticides cause vestibular alterations through a slow and silent intoxication. CONCLUSIONS: medical care for this population exposed to neurotoxic substances require an interdisciplinary approach, to guarantee efficient preventive and therapeutic measures.

Os autores relatam três casos de varíola bovina em humanos, ordenhadores manuais em vacas infectadas, na microrregião de Itajubá, MG. As técnicas diagnósticas foram: isolamento de amostra semelhante ao vírus vaccinia de secreções das lesões cutâneas, reação em cadeia de polimerase, microscopia eletrônica e anticorpos para Orthopoxvirus no sangue dos pacientes.


The authors report three human cases of cowpox infection, among farm workers who were manually milking infected cows, in the microregion of Itajubá, Minas Gerais. The diagnostic techniques used were: isolation of samples similar to the vaccinia virus, from skin lesion secretions; polymerase chain reaction; electronic microscopy; and antibodies for Orthopoxvirus in the patients' blood.

ABSTRACT

BACKGROUND/AIMS: We assessed twelve cases of suspected chronic pesticide intoxication, with medically unexplained physical symptoms. METHODS: Complete blood cell count (CBC), blood chemistry, routine urinalysis, chest X-ray, ECG, gastrofiberscopy, abdominal ultrasonography, neuroselective sensory nerve conduction threshold, and psychological assessment were performed on 12 farmers who believe themselves to have suffered from chronic pesticide intoxication. RESULTS: No specific abnormalities were observed on CBC, routine urinalysis, chest X-ray, ECG, gastroscopy, abdominal ultrasonography, or peripheral nerve conduction velocity test. They persistently manifested helplessness, depression, and anxiety. The results of both psychological assessment and general physical examination revealed the following clinical features: depression (8 cases), multiple chemical hypersensitivity syndrome (2 cases), alcoholism (1 case), and religious preoccupation (1 case). CONCLUSION: In those living in the western rural area of South Korea, depression is a prominent ongoing presentation in pesticide-exposed farmers, in addition to unexplainable physical symptoms.

Objetivo- Detectar los efectos citotóxicos y genotóxicos en trabajadores agrícolas, mediante estudios de biomonitoreo genético. Diseño. Casos y controles. Se realizó un estudio para conocer los niveles de actividad de colinesterasa en los trabajadores del ICA Tibaitatá, y su correlación con la exposición ocupacional a plaguicidas inhibidores de colinesterasa (Organofosforados y Carbamatos). Se tuvieron en cuenta también las siguientes variables: sexo, edad, uso de elementos de protección personal, antiguedad (Tiempo de exposición), ocupación, exposición extraocupacional, embarazo, lactancia, menstruación y presencia de náuseas y síntomas como vómito. El centro de investigación de Tibaitatá, el mas grande del ICA, está ubicado en la sabana de Bogotá y allí se desarrollan trabajos de investigación agrícola y pecuaria que implican exposición a plaguicidas en diversas labores. Las determinaciones de colinesterasa se hicieron por el método colorimétrico de campo, con muestra de sangre capilar y utilizando el equipo de levibond. se consideran anormales o bajos los niveles inferiores al 75 por ciento. Se tomo una muestra de 323 trabajadores, de los cuales 95 (29,4 por ciento) estaban directamente expuestos en su trabajo a plaguicidas inhibidores de colinesterasa. A estos se les aplicaron dos encuestas para conocer las variables y luego se les tomo muestra de sangre capilar, la cual fue procesada en el mismo lugar. Para comprobar la exposicion ocupacional a plaguicidas, se hizo una visita de inspeccion a las distintas dependencias. ABSTRACT

Para conocer la incidencia y cuadro clínico del dengue ocurrida en los meses de Marzo a Setiembre de 1990, se desarrolló el presente trabajo en el departamento de Medicina del Hospital de Apoyo de Tarapoto, en 136 pacientes hospitalizados. El 63.23 por ciento fueron mujeres siendo la edad promedio de 33,54 +/- 12,36 años. El grupo etáreo más frecuentemente afectado estuvo entre los 20 y 29 años (38,96 por ciento). El 76,47 por ciento se hospitalizaron en Mayo y Junio. El 81,63 por ciento procedían de la provincia de San Martín. La ocupación más frecuente fue ama de casa (32,29 por ciento) y empleados (28,67 por ciento). El promedio de tiempo de enfermedad fue de 4,77 +/- 2,85 días. El cuadro clínico estuvo constituído mayormente por fiebre (100 por ciento), dolores osteomusculares (77,94 por ciento), náuseas/vómitos (69,12 por ciento), cefalea severa (61,91 por ciento), astenia (41,16 por ciento), sangrado patológico (33,82 por ciento) y anorexia (21,32 por ciento). Los pacientes fueron internados con diagnóstico inicial de virosis (35,29 por ciento), Dengue (28,68 por ciento), síndrome febril (10,29 por ciento) y gastroenterocolitis aguda (4.41 por ciento). Los pacientes recibieron al inicio analgésicos antipiréticos (72,05 por ciento), antieméticos (55,88 por ciento), fluidos endovenosos (35,29 por ciento) y antiácidos en 33,82 por ciento. El período sintomático fue de 4,03 +/- 2,45 días
